These resources were carefully selected specifically for our community-based providers in conjunction with HHC clinicians and are available to all physicians who partner with HHC Connectx.  The main disease areas highlighted are: Asthma, Diabetes, Hypertension, Depression, and Pediatric Nutrition. 

The site includes clinical guidelines, patient education materials (translated into several languages), patient self-management tools, and relevant links from a variety of sources including NYC HHC, MetroPlus Health Plan, and New York City Department of Health.
